BakgrunnTraume er i dag en av de viktigste årsakene til død og uførhet hos en ellers ung og frisk befolkning. Dette er også en pasientgruppe med store kostnader for samfunnet, knyttet til dyr behandling med lange sykehusopphold, intensivbehandling og kirurgi. Nasjonalt kompetansesenter for traumatologi har lansert en Nasjonal traumeplan med anbefalinger til hvordan traumepasienter best kan håndteres. Denne studien vil undersøke hvorvidt anbefalingene som er gitt i traumeplanen vedrørende rehabilitering følges opp ved St. Olavs Hospital. Pasientgruppen er i dette materialet fokusert på multitraumatiserte pasienter med ortopediske skader, uten traumatisk hjerneskade eller ryggmargskade.
MetodeLokal traumeregistrar ved St. Olavs Hospital hentet ut oversikt over alle pasienter mottatt ved St. Olavs Hospital i 2015 og 2016 med en ISS over 15. Totalt 58 pasienter ble inkludert. Ut fra listen kan pasientene identifiseres med fødsels- og personnummer. Ved journalgjennomgang er det kartlagt i grove trekk hvor mye kirurgi og intensivbehandling, samt hvilke tiltak som ble gjort under oppholdet med tanke på rehabilitering. Det er også kartlagt hvilket tilbud pasientene ble skrevet ut til.
KonklusjonTil tross for resurskrevende og intensiv primærbehandling av disse pasientene, er det en usystematisk og lite planlagt håndtering av rehabilitering av disse pasientene. Funnene tilsier at St. Olavs Hospital per i dag ikke følger opp anbefalingene i traumeplanen vedrørende rehabilitering. Anbefalingen er å implementere dette snarest mulig. BackgroundTrauma is one of the leading causes of death and disability to an otherwise young and healthy population. This patient group cause high socio-economic costs to society, based on expensive treatment with long hospital stays, intensive care treatment and surgery. National centre of competence on Traumatology has released a National trauma plan, giving recommendations for how to best manage trauma patients. This study examines whether the given recommendations regarding rehabilitation is being adhered to at St. Olavs Hospital. In this material the main patient group is the multi trauma patient with orthopaedic injuries, excluding patients with a traumatic brain injury or spinal cord injury.
MethodThe local trauma registrar at St. Olavs Hospital made an overview of all patients received at St. Olavs Hospital in 2015 and 2016 with an ISS over 15. A total of 58 patients were included. From the list the patient could be identified by their national identity number. Reading through patient records to make an outline of how much surgery and intensive care each patient received, and which measures were taken during the hospital stay to ensure rehabilitation. Where the patients were discharged to was also registered.
ConclusionDespite expensive and intensive primary treatment of these patients, it is an unsystematic approach and no planned management of rehabilitation for this patient group. The findings in this study indicates that St. Olavs Hospital does not adhere to the recommendations given in the national trauma plan in regard to rehabilitation. We recommend implementing the necessary changes to meet these guidelines.
