dc.description.abstractBakgrunn: Den teknologiske utviklingen og nyfødtmedisin har kommet langt de siste årene og stadig flere premature barn født i uke 23-24 vokser opp uten komplikasjoner. I Norge ligger antallet premature fødsler på ca. 5% av alle barn som fødes årlig. Et opphold på nyfødt intensiv (NISE) eksponerer de premature spedbarna for ulike typer stimuli i form av lys, lyd, smerte og uro. Stimuli som kan føre til forstyrrelser av normal vekst og utvikling på grunn av økt stress og medfølgende belastning på umodne organsystemer. Støynivået på NISE er slik stimuli som kan være skadelig. Hensikt: Hensikten med studien er å kartlegge sykepleieres opplevelse av støykilders innvirkning på premature ved nyfødtintensiv og kartlegge sykepleieres opplevelse av støyreduserende tiltaks effekt på støynivået. Metode: En kvantitativ spørreundersøkelse ble utført i samarbeid med nyfødt intensiv avdeling ved St. Olavs Hospital der 34 sykepleiere deltok. Undersøkelsen ble utført under covid-19 pandemien i 2020. Resultater: 86 % av sykepleierne mente premature nyfødte påvirkes av støynivået ved NISE. Støykildene som påvirket de premature mest, var pustestøtteapparater og håndtering av dører/skap. Tiltakene med best effekt på støynivået var «bevisst bruk av stemmeleie», «NIDCAP-metoden» og «bevist utførelse av prosedyrer/undersøkelser». Tiltaket med minst effekt var «lyddemping til nyfødte». Terapeutisk lydstimulering var et ukjent fenomen for mange. Undervisning og informasjon til helsepersonell viste å ha en hensiktsmessig virkning på sykepleieres atferd vedrørende støynivå. Konklusjon: Studien viste at støynivå er et viktig fokusområde på NISE og at de fleste ansatte jobber bevisst for å redusere støynivået. Sykepleierne rapporterte at støynivået påvirket premature til en viss grad. Enkelte støyreduserende tiltak ble ikke tatt i bruk på avdelingen og viste liten effekt på støynivået. Oppsummert kan resultatene i studien indikere et behov for tilrettelegging av undervisning om støynivå og støyreduserende tiltak, som gir en kunnskapsbasert praksis og opprettholder bevissthet rundt støynivå i arbeidshverdagen. dc.description.abstractBackground: In recent years, technological developments and newborn medicine have come a long way. An increasing number of preterm infants born before week 23-24 survive without further complications. In Norway, the number of preterm births is approximately 5% of all children born annually. A stay in the NICU exposes the preterm newborn to various types of stimuli, such as noise, that may cause disruption of normal growth and development, due to increased stress levels and pressure on immature organ systems. The aim: The purpose of this study was to chart how nurses experience the impact noise have on preterm infants in the NICU and to investigate nurse experience of noise reducing interventions in the NICU. Methods: A quantitative survey was conducted in cooperation with the NICU at St. Olavs Hospital, where 34 nurses participated. The survey took place during the covid-19 pandemic of 2020. Results: 86% of the nurses participating thought that premature infants are affected by the noise levels at NICU. The most disturbing noise sources were respiratory aids and handling of doors/cabinets. The most effective interventions were “conscious use tone of voice”, “NIDCAP” and “attentive execution/performance of procedures/examinations”. The least effective measure was “earmuffs”. “Therapeutic sound stimulation” was an unknow phenomenon. Teaching healthcare professionals proved to influence nurses’ behavior regarding the noise levels. Conclusion: In conclusion, the study reveals that noise levels are an important focus area at the NICU, and most employees strive to reduce noise levels. The participants reported that noise level affected the preterm infant to some extent. Some noise-reducing measures where not implemented at the department and showed little effect on the noise level. In summary, the results may indicate a need for facilitation of teaching noise levels and noise-reducing measures, which provides a knowledge-based practice and maintains employee awareness surrounding noise levels.
