dc.description.abstractBakgrunn: 15-20% av personer med diabetes vil utvikle et diabetisk fotsår. Disse sårene er vanskelige å få til å gro på grunn av negative patologiske prosesser og betegnes derfor som et kronisk sår. For å få et diabetisk fotsår til å gro er riktig behandling essensielt, og man er derfor avhengig av gode vurderinger og god sårdokumentasjon. Hensikt: Hensikten med oppgaven er å belyse sykepleiers behandlende funksjon av diabetiske fotsår i hjemmesykepleien, herav viktigheten av strukturerte sårvurderinger, sårbehandling og dokumentasjon. Metode: I denne oppgaven er det brukt litteraturstudie som metode. Oppgaven inneholder kunnskap som er innhentet ved hjelp av strukturerte søk i ulike databaser for å finne eksisterende kunnskap og forskning om temaet. Vi har valgt 6 vitenskapelige artikler til denne oppgaven. Resultat: Basert på funn i artiklene er det fire temaer som er viktige for å sikre optimal sårtilheling: Sykepleiers rolle i tverrfaglig samarbeid, sykepleiers kompetanse i sårbehandling, strukturert sårdokumentasjon og oppfølging ved bruk av telemedisin. Konklusjon: Sykepleiere i hjemmesykepleien må øke sin kompetanse om sårbehandling av diabetiske fotsår. Ved bruk av telemedisin kan man tilrettelegge for bedre oppfølging og kommunikasjon mellom primær- og spesialisthelsetjenesten. Telemedisin legger også tilrette for strukturert sårdokumentasjon, noe som er viktig for å oppnå optimal sårtilheling.
dc.description.abstractBackground: 15-20% of people with diabetes may develop a diabetic foot ulcer. Those wounds are hard to heal due to the negative pathological processes and is therefore referred to as chronic wounds. To heal a diabetic foot ulcer, the right treatment is essential and the treatment relies on structured assessments and good wound documentation. Objective: The purpose of the task is to illuminate the nurses therapeutic function of diabetic foot ulcer in home care, hence the importance of structured wound assessments, wound care and documentation. Methods: In this assignment, literature study is used as a method. The assignment contains knowledge whos been acquired through structured searching in databases to find existing knowledge and research on the topic. In this study we have chosen six scientific articles. Results: Based on findings in the articles, there are four themes that are important for ensuring optimal wound healing: Nurse's role in interdisciplinary collaboration, nurse's expertise in wound care, structured wound documentation and follow-up using telemedicine. Conclusions: Nurses in home nursing must increase their expertise in wound care for diabetic foot ulcers. The use of telemedicine can facilitate better follow-up and communication between primary and specialist health services. Telemedicine also facilitates structured wound documentation, which is important for achieving optimal wound healing.
