HVORDAN KAN SYKEPLEIER BIDRA TIL AT PASIENTEN IVARETAR SIN SEKSUALITET ETTER EN STOMIOPERASJON?

Abstract

Bakgrunn: En stomi er en kirurgisk åpning i kroppen, som legges ut fra et hulorgan og ut på huden. Det foretas rundt 2000 stomioperasjoner årlig. Det er et alvorlig inngrep, og påvirker både livssituasjonen, seksualiteten og selvbildet til pasienten. Det er viktig at sykepleiere bidrar ved å gi tilstrekkelig informasjon og veiledning både før og etter en stomioperasjon.

Hensikt: Hensikten med denne studien er å belyse hvordan sykepleier kan bidra for at pasienten skal ivareta sin seksualitet etter en stomioperasjon.

Metode: Dette er en litteraturstudie som bygger på fag- og forskningsbasert kunnskap. Det er foretatt strukturerte søk i databasene Pubmed, Medline Ovid og Cinahl. Det er inkludert både kvantitativ og kvalitativ forskning.

Resultat: Resultatenes funn synliggjør at holdninger, kunnskap og opplæring blant sykepleiere er viktige faktorer for å kunne bidra til at pasienten ivaretar sin seksualitet. Pasientens selvbilde kan bli endret etter en stomioperasjon, og hvordan sykepleiere møter pasienten vil være av stor betydning. Tilstrekkelig informasjon og god veiledning fra sykepleiere er avgjørende for hvordan pasienten takler den nye tilværelsen. Sykepleiere kan veilede pasienter om seksualitet ved bruk av PLISSIT-modellen.

Konklusjon: Etter en stomioperasjon er det stort fokus på stell og pleie av en stomi. For å kunne ivareta både det fysiske og psykiske rundt pasienten, må sykepleiere øke fokuset på seksualitet og seksuelle bekymringer etter en stomioperasjon. Funnene viser at kunnskap og holdninger hos sykepleiere er et viktig grunnlag i utøvelsen av helhetlig sykepleie.

Background: An ostomy is a surgical opening in the body, that is laid out from a hollow organ and out through the skin. Around 2000 ostomy operations is performed every year. It is a serious procedure that affects both the patients life situation, their sexuality and self-image. It is important that nurses help the patient by providing adequate information and guidance before, as well as after an ostomy operation.

Purpose: The purpose of this study is to shed light on how a nurse can help the patient to maintain his sexuality after an ostomy operation.

Method: This is a literature study based on subject- and scientific knowledge. Structured searches are made in Pubmed, Medline Ovid and Cinahl databases. Both quantitative and qualitative research are included.

Results: The findings reveal that attitude, knowledge and the training received among nurses are important factors in helping the patient maintain their sexuality. The patients self-image may change after an ostomy operation, and the way the nurse meets the patient will be highly important. Adequate information and guidance from nurses are crucial to how the patient copes with the changes. Nurses can educate patients on sexuality by using the PLISSIT-model.

Conclusion: After an ostomy operation there is paid great attention to how to handle and care for the ostomy. In order to take care of both the physical and mental aspects of the patient, nurses must focus on the patients sexuality and sexual concerns post surgery. The findings reveal that the knowledge and attitude of nurses is an important base in the practice of comprehensive nursing.
