Når kreften er uhelbredelig; Hvordan kan sykepleier gjennom bruk av god kommunikasjon, ivareta pasienten i krise ved overgang til palliativ fase?

Abstract

Bakgrunn: Kreftpasienter i overgangen til palliativ fase kan oppleve ulike krisereaksjoner. Derfor er det viktig at sykepleier har kunnskap om hvordan en kan bruke målrettet kommunikasjon for å ivareta pasienten i krise.

Hensikt: Belyse hvordan sykepleier gjennom/ ved bruk av god kommunikasjon kan ivareta pasienter med uhelbredelig kreft i overgangen til palliativ fase.

Metode: Oppgaven er en litteraturstudie, som bygger på vitenskapelig forskning relevant for sykepleiefaget. Artiklene er inkludert basert på relevans for problemstillingen. Strukturerte søk er utført ved bruk av databasene CINAHL og PubMed. Søkeord: oncology nursing, nurse-patient relations, communication skills, cancer patient, information, truth disclosure, prognosis, terminally ill patients.

Resultat: Det er flere ulike faktorer som påvirker hvordan sykepleier kommuniserer og ivaretar kreftpasienten, som; erfaringer, holdninger, ferdigheter, og relasjon til pasienten. Noen viktige faktorer sykepleier må være bevisst ved ivaretakelsen av kreftpasienten er; forberedelse til samtalen, måten informasjonen formidles, balanseringen av håp og sannhet, og om informasjonen blir forstått av pasienten.

Konklusjon: Ved ivaretakelse av pasienter i krise er det flere viktige faktorer sykepleier må være bevisst; tidlig kartlegging av den palliative fasen, hjelpe pasienten å akseptere sannheten og sikre forståelse av informasjonen pasienten mottok. Det er viktig at sykepleier er profesjonell, forståelsesfull, omsorgsfull og empatisk.

Background: Cancer patients in transition to palliative care experience different reactions in crisis. That’s why it’s important for nurses to have knowledge of how to use targeted communication to care for patients in crisis.

Aim: To shed light on how the nurse through good communication can take care of patients with incurable cancer in the transition to palliative care.

Method: This study is a literature review, that uses scientific research related to the nursing profession. The articles were included based on the aim of this study. The structured search is done by using the databases CINAHL and PubMed. Keywords: oncology nursing, nurse-patient relations, communication skills, cancer patient, information, truth disclosure, prognosis, terminally ill patients.

Result: There are multiple factors that affects how the nurse communicates and cares for the cancer patient, like; experiences, opinions, skills, and patient-relations. Some important factors nurses must be aware of when caring for cancer patients; preparations for the conversation, how the information is conveyed, balancing hope and truth, and if the information is understood by the patient.

Conclusion: When caring for patients in crisis there are several important factors nurses must keep in mind; gathering data early in palliative care, helping patients to accept the truth and making sure the patient understands the information that is given. It’s important that the nurse is professional, understanding, caring and empathic.
