dc.description.abstractFormålet med denne oppgaven var å finne ut av hvorfor det kan være hensiktsmessig å benytte genotypisk resistensbestemmelse sammen med fenotypisk resistensbestemmelse av linezolidresistens hos Meticillinresistente Staphylococcus aureus (MRSA). I sammenheng med den økende trusselen MRSA har i samfunnet i dag, har det blitt lagt fokus på fordeler og ulemper ved fenotypiske og genotypiske resistensbestemmelser. I oppgaven svares det på problemstillingen: Hvorfor er det en fordel å benytte genotypisk resistensbestemmelse av linezolid hos MRSA? For å finne ut av dette ble det utført en litteraturstudie. Hovedfokuset ble lagt på Staphylococcus Aureus, MRSA, mekanismene som fører til antibiotikaresistens og de ulike metodene for resistensbestemmelse. Litteratursøket bestod hovedsakelig av å lese seg opp på nødvendig teori ved bruk av ulike databaser, faglige lærebøker og relevante nettsider. Resultatet belyser utfordringer og uoverensstemmelser spesifikt til fenotypiske metoder ved deteksjon av linezolidresistens hos MRSA og Staphylococcus aureus generelt. Det blir også diskutert fordeler og ulemper ved bruk av polymerase kjedereaksjon (PCR) som genotypisk metode for å påvise de ulike genene som koder for linezolidresistens. Litteraturstudien konkluderer med at å benytte og øke bruken av genotypiske metoder vil kunne bidra til bedre kontroll av linezolidresistens hos MRSA. Dette kan gi bedre og enklere kontroll over forekomsten av resistensgenene cfr, poxtA og optrA, og dermed resistensutviklingen hos MRSA. Antibiotikaresistens kommer ikke til å forsvinne eller minske av seg selv. Vi håper at med nye opplysninger og bedre verktøy vil veien mot å benytte genotypiske resistensbestemmelser satses mer på i framtiden i kampen mot antibiotikaresistens.
dc.description.abstractThe purpose of this thesis is to outline the use of genotypic susceptibility testing in addition to phenotypic susceptibility testing for linezolid resistance in Methicillin resistant Staphylococcus aureus (MRSA), and why this would be suitable. Taking into context the increasing threat that MRSA poses to today’s society, both the advantages and disadvantages of phenotypic and genotypic susceptibility testing were of direct focus. This report seeks to answers the question: Why is it advantageous to utilize genotypic susceptibility testing of linezolid in MRSA? To do this, a literature review was conducted with a focus on Staphylococcus aureus, MRSA, as well as the mechanisms that lead to antibiotic resistance and the various methods used in antibiotic resistance testing. The literature examined consisted largely of necessary theory found using various databases, textbooks, and relevant websites. The results of the review highlight the challenges and discrepancies specific to phenotypic methods in the detection of linezolid resistance in MRSA and Staphylococcus aureus in general. The advantages and disadvantages of using the polymerase chain reaction (PCR) as the preferred genotypic method for detection of various genes encoding for linezolid resistance were also discussed. This literature review concludes that together, the utilization of PCR and at the same time increasing the use of genotypic methods, will be able to contribute to better control of linezolid resistance in MRSA. This can lead to improved and more manageable control over the presence of the specific resistance genes cfr, poxtA, and optrA, and thus resistance development within MRSA. Antibiotic resistance will not slow down or disappear on its own. We hope that with new information and improved tools, that genotypic susceptibility testing will be of stronger focus in the coming years in the fight against antibiotic resistance.
