dc.description.abstractDenne masteroppgaven omhandler NIPT (non-invasive prenetal testing), en fosterdiagnostisk metode som kan undersøke trisomier hos foster ved hjelp av en blodprøve fra mor. Jeg studerer implementeringen av denne testen inn i det fosterdiagnostiske systemet eller regimet i Norge. Tidsmessig strekker oppgaven seg fra 2012, da den første søknaden om å anvende denne teknologien ble skrevet, og til høsten 2018 – rett før blodprøven ble anvendt ved norske sykehus. Blodprøven ble godkjent av Helse og omsorgsdepartementet i mars 2017. Oppgaven analyserer hvordan en teknologi som NIPT blir rammet inn og dermed «konstruert» av ulike aktører, og hvordan dette påvirker blant annet politiske beslutninger rundt anvendelse og regulering av teknologien. Jeg bruker Callons innrammingsperspektiv for å belyse hvordan ulike aktører skaper ulike forståelser av samme teknologi, og hvordan dette påvirker bruken av den. Oppgaven består av tre analysekapitler, hvor det første (kapittel 4) analyserer den faglig-politiske prosessen rundt innføringen av NIPT. Denne handlet om at ulike aktører skulle gi uttalelser og råd. Jeg fant at det gjennom denne prosessen ble produsert en slags «sammenveving» av tidligere innramminger av fosterdiagnostiske verktøy (som behandlings- eller sorteringsteknologier). Da helseminister Høie tok den endelige beslutningen kan vi si han hadde «lært» av tidligere fosterdiagnostiske debatter, og ønsket å ta innover seg de eksternalitetene som tidligere hadde ført til politisk uro og strid. Det andre analysekapittelet (kapittel 5) er en medieanalyse av den offentlige debatten om NIPT. Her fant jeg ikke den samme tendensen til konvergens som i den faglig-politiske prosessen. Mediedebatten viste også at Høies innramming av NIPT ikke var stabil eller holdbar nok til å motstå debatt. Oppgavens siste analysekapittel tar for seg tiden etter godkjenningen, og etableringen av en infrastruktur for prøvetaking og analyse av prøvene. Dette kapittelet handler også om noen sentrale fostermedisineres innramming av NIPT – en forståelse konstruert av de som gjør jobben «på gulvet», og står ansikt-til-ansikt med de gravide som skal eller ikke skal testes. Jeg fant her at både arbeidet med infrastruktur, og fostermedisinernes utfordringer var sterkt preget av særlig økonomiske, tekniske og praktiske elementer. Etikken som var et omdreiningspunkt i kapittel 4 og 5, var ikke like tilstedeværende her – annet enn gjennom de begrensningene som ble lagt gjennom godkjenningen, som ble en av retningslinjene for legenes videre praktisk arbeid. Fostermedisinerne var særlig opptatt av kvinnenes selvbestemmelse.
dc.description.abstractIn this master thesis I study the implementation of NIPT (non-invasive prenatal testing) into the fetal diagnostic system in Norway. The NIPT test is a method in prenatal diagnosis that can examine trisomies in a fetus with the help of a blood test from the mother, and was approved by the Ministry of Health and Care Services in March 2017. In this thesis I study the efforts to implement the test in the time span between 2012, when the first application about applying the technology was written, until the autumn of 2018 – just before the blood test started being used in Norwegian Hospitals. The thesis analyzes how a technology such as NIPT is affected and thus "constructed" by different actors, and how this affects, among other things, political decisions about the use and regulation of the technology. I use Callon's framing perspective to illustrate how different actors create different understandings of the same technology, and how this affects its use. The thesis consists of three analysis chapters, where the first (chapter 4) analyzes the knowledge produced to inform decision makers and the political process regarding the introduction of NIPT, where different actors gave statements and advice into the public administrative process. I found that existing frames of embryonic diagnostic tools and previous fetal diagnostic debates influenced the political decisions regarding NIPT, causing Bent Høie to absorb the externalities that had previously led to political unrest and strife, into his final decision. The second analysis chapter (chapter 5) is a media analysis of the public debate on NIPT, where I did not find the same tendency towards convergence as in the professional-political process. The media debate also showed that Høie's framing of NIPT was not stable or durable enough to withstand debate. The final chapter analyzes the time after the approval, and the establishment of an infrastructure for the use of NIPT. This chapter also deals with how central fetal medicine specialists frame NIPT. I found that both the work on infrastructure and the challenges of fetal medicine were strongly influenced by particularly economic, technical and practical elements. The ethics that was a pivotal point in chapters 4 and 5 were not as present here, and the fetal medicine specialists were on the other hand particularly concerned with the women's right of self-determination.
