dc.description.abstractNORSK: Bakgrunn: I praksis som student, opplevde jeg at noen anestesisykepleiere brukte søvndybdemonitorering under generell anestesi på pasienter med kjent hjerte- og karsykdom, for å bidra til å forebygge komplikasjoner som awareness og hypotensjon. Andre anestesisykepleiere mente at søvndybdemonitorering ikke er nøyaktig nok, og at det har liten nytte. De mente at det bare ble enda et parameter å følge, og at anestesisykepleiere ikke lengre lærer seg, eller våger å stole på klinikken. Awareness er en fryktet, sjelden, men svært alvorlig komplikasjon. Bruken av søvndybdemonitorering og dens troverdighet, opplevde jeg som svært omdiskutert blant anestesisykepleierne. Derfor ønsker jeg at oppgaven skal bidra til å belyse temaet awareness og søvndybdemonitorering, og redegjøre for hva forskning og faglitteratur sier om insidens, årsaks- og risikofaktorer og forebygging av awareness, og belyse forskningens ståsted på søvndybdemonitorering. Oppgaven er avgrenset til å se på den best dokumentert typen, Bispectral Index (BIS). Metode: Oppgaven er et litteraturstudie som søker å finne svar på problemstillingen via forskning og faglitteratur rundt temaet. Den bruker elementene i kunnskapsbasert praksis. Det er gjort systematisk søk i PubMed, og usystematiske søk i google scholar. Det er valgt ut 19 artikler fra disse søkene, som er satt inn i en litteraturmatrise, i tillegg til relevant teori fra pensumlitteraturen. Resultat:Forskningen viser en insidens av eksplisitt awareness på 1 – 2 av 1000 ved alle generelle anestesier, mens hos pasienter som har økt risiko kan den komme opp mot 1% . Disse er pasienter med kjent hjerte- kar sykdom, akutt kirurgi, de som har fått små doser inhalasjonsanestesi. Bruk av lystgass og TIVA, og når tekniske problemer eller menneskelig feil oppstår under anestesien øker også risikoen. Awareness kan være en traumatisk opplevelse, og ca 1/3, får psykologiske konsekvenser. 6 av 9 store studier støtter bruken av BIS monitorering for å bidra til å redusere forekomsten av awarness hos risikopasienter, 2 finner ingen forskjell og 1 studie avviser støtten til bruk av BIS ved inhalasjonsanestesi, men støtter ved bruk av TIVA. Samtlige oversiktsartikler konkluderer med at det må forskes videre på søvndybdemonitorering, siden det er utilfredstillende forskningsbevis og statistisk relevans på at søvndybde monitorering kan tas inn som rutinemonitorering i den kliniske praksis. Resultatene viste imidlertid at BIS monitorering har dokumentert effekt, og kan nyttes som et valg for å optimalisere overvåkingen av pasienter med økt risiko for awareness. Konklusjon:Søvndybdemonitorering bør brukes som et hjelpemiddel i en multimodal monitorering i sammen med klinikken hos pasienter med kjent hjerte- og karsykdom. Samlet vil dette føre til at det anestetiske forløp i innlednings- og vedlikeholdsfasen gjennomføres så opptimalt som mulig på tross av pasientens økte risiko for komplikasjonen awareness. BIS verdien kan brukes som en bekreftelse på at de kliniske parameter og tegn samsvarer med det forventede, dette vil bidra til å øke pasientsikkerheten og gjøre det mulig for anestesisykepleier å dokumentere søvndybden, via BIS verdien i anestesijournalen, som vil være med på å kvalitetssikre den kliniske praksis.no_NO
dc.description.abstractENGLISH: Background: In student practice I experienced that some of the anesthetic nurses used EEG based monitoring in patients who had known cardiovasculair diseases, to prevent complications as awareness and hypotention during general anesthesia. But there where other anesthetic nurses that ment this was not a sure methode, and that this was not useful. They said that this gave them only one more parameter to follow, and that this led anesthetic nurses to no longer learn and trused their clinical views. Awareness is a feard, rarely but very seriouse complication. I experienced that the use of EEG based monitoring and its credibility was highly debated among the anesthetic nurses. That is way I wanted to write this study to illustrated these two issues, awareness and EEG based monitoring, and account what research and literature say about insidens, causal- and risk factors and prevention of awareness, and to see research standpoint to EEG based monitoring. This study is looking at the best documented one, the Bispectral Index (BIS). Methodes:This is a literature study that seeks to find the answer to this issue through research and literature. The study uses elements in evidence-based practice in this search, and have done a systematic search in PubMed, and a nonsystematic search in google scholar. 19 articles of studies from this searches, as well as theory from relevant literature in connection with further education in anesthesia nursing have been placed into a literature matrix in this document, and is the basis of this study. Results:Researchers found an insidens of explisitt awareness on 1–2 of 1000 general anesthesia, but in high risk patiens the insidens can be up to 1 %. High risk patients are those with cardiovascular diseases, emergency surgery, those who have got small doses of inhalation anesthetics. The use of nitrous oxide and TIVA, and those who had technical problems or human error during the anesthesia can also have a higher risk. Awareness can be a traumatic experience , about 1/3 of those who experienced it got psychological consequenses. 6 av 9 large studies support the use of BIS monitoring regarding the issue of its ability to prevent awareness during general anesthesia in high risk patients. 2 studies find no differences in using BIS and 1 study rejects the support regarding to anesthesia with inhalation, but support BIS monitoring when TIVA is used. All review articles concludes that further researches on BIS is needed, since the reseach evidence not yet show a statistically significant to take it in as a routine monitoring in clinical practice. The results showed the BIS monitor`s credibility, and that it can be used as a choice to optimize in patients at high risk of awareness. Conclusions: BIS can be used as an aid in a multimodal monitoring toogether with clinical observations in patients at high risk of awareness. This will optimize anesthetic proceeding during induction and maintance despite patients high risk of the complication awareness. The BIS value can be used as a confirm that the clinical parameter and signs are as expected regarding to depth of anesthesia. This will help to increase the patient safety, and enable the anesthetic nurse to document the depth of anesthesia in the anesthetic journal, and by this ensure the quality of clinicale practice.no_NO
