Sammendrag
BakgrunnEmosjonelt ustabil personlighetsforstyrrelse (EU PF) er en alvorlig og gjennomgripende diagnose preget av utfordringer med emosjonsregulering. Personlighetsforstyrrelsen fører til nedsatt funksjon for pasientene. Systems Training for Emotional Predictability and Problem Solving (STEPPS) er et 20-ukers manualisert, detaljert og strukturert gruppebasert behandlingstilbud for pasienter med EU PF. Oppgaven tar sikte på å utforske hvordan STEPPS-behandling påvirker emosjonsreguleringsvansker, samt undersøker sammenhengen mellom emosjonsregulering og funksjon.
Materiale og metode Studiedesignet er en kvantitativ, prospektiv kohortstudie som ser på behandlingseffekt med et pre-post design. Utvalget består av 220 pasienter diagnostisert med BPD eller med trekk av diagnosen. En rekke måleverktløy ble brukt for å besvare forskningsspørsmålene. Emosjonsreguleringsvansker ble målt med Borderline Evaluation of Severity Over Time (BEST), mens funksjon ble målt med Work and Social Adjustment Scale (WSAS). Verdiene rapportert før behandling og etter behandling (uke 20) ble benyttet i parede t-tester og lineære regresjonsanalyser.
ResultaterParede t-tester av både «fullført behandling»-utvalget og «Intention-to-treat»-utvalget viste signifikant forbedring i emosjonsreguleringsvansker fra før behandling til etter behandling. Forbedringen ble observert for både totalskåren i BEST og for de tre subbolkene. Regresjonsanalysen antydet at økt selvrapportert forbedring i emosjonsregulering var assosiert med endring i funksjonsnivå.
Konklusjon Funnene støtter behandlingseffekten STEPPS har på emosjonsreguleringsvansker, i tillegg til å styrke evidensen for sammenhengen mellom emosjonsregulering og funksjon. Oppgaven gir et grunnlag for videre forskning på STEPPS i norske forhold. BackgroundEmotionally unstable personality disorder (EU PD) is a severe and pervasive diagnosis characterized by challenges in regulating emotions, leading to impaired functionality in patients. Systems Training for Emotional Predictability and Problem Solving (STEPPS) is a 20-week long manualized, detailed, and structured group-based treatment for patients with EU PD. This study aims to explore the effect STEPPS-treatment has on emotion regulation difficulties, in addition to examining the relationship between emotion regulation and functionality.
Material and method The study design is a prospective cohort study of a quantitative nature, utilizing a pre-post design to investigate treatment effects. The sample consists of 220 patients diagnosed with BPD or exhibiting traits of the disorder. Emotion regulation difficulties were measured using the Borderline Evaluation of Severity Over Time (BEST), while functionality was measured using the Work and Social Adjustment Scale (WSAS). Values reported before and after treatment (week 20) were analyzed using paired t-tests and linear regression analyses.
ResultsPaired t-tests for both the completed treatment sample and the Intention-to-treat sample revealed a significant improvement in emotion regulation difficulties from before treatment to after treatment. Improvement in the BEST scores was observed across total scores and all subcategories. Regression analysis suggested that increased self-reported improvement in emotion regulation was associated with changes in functional levels.
Conclusion The findings support the treatment effect that STEPPS has on emotion regulation difficulties, further reinforcing the evidence of the relationship between emotion regulation and functionality. The study provides a foundation for further research on STEPPS in the context of Norwegian settings.
