Valg av administrasjonsmåte til barn - Sykepleieres erfaringer («FAGARTIKKEL»)

Sammendrag
Bakgrunn/hensikt Barn i akuttmottak kan ofte være engstelige og preget av smerter. I noen tilfeller vil det være behov for medikamentell behandling av smerte og engstelse hos disse barna. Sykepleiere administrerer legemidlene, som kan variere i administrasjonsmåte. Hensikten med denne studien er å undersøke hvilke faktorer sykepleiere i akuttmottak opplever påvirker valget av administrasjonsmåte til barn.
MetodeOppgaven ble utarbeidet med kvalitativ metode med fenomenologisk tilnærming, og gjennomført av to studenter. Det ble tatt i bruk individuelle semistrukturerte intervju, med totalt fire sykepleiere ansatt ved et akuttmottak for barn. I forkant av intervjuene ble det utsendt informasjonsskriv, og intervjuene ble gjennomført ved hjelp av en intervjuguide. Intervjuene ble nedskrevet, og analysert med inspirasjon av Dallands metode for analyse og tolkning, og tematisk bearbeiding.
ResultaterSykepleiere opplever at kommunikasjon og samarbeid, valgmuligheter, pasientsituasjon og tidspress var faktorer som påvirket hvilken administrasjonsmåte som ble valgt. Sykepleierne kommuniserte og samarbeidet med pasient, foreldre og lege for å kartlegge hvilken administrasjonsmåte som var best egnet til den enkelte pasienten. Hvilke legemiddelformer som var tilgjengelige og pasientens tilstand påvirket hvilken administrasjonsmåte som ble ansett som hensiktsmessig. Tidspress forårsaket av bemanningsproblematikk, pasientpågang og videreutviklinger i digitale systemer påvirket også sykepleiernes arbeid i valgprosessen.
KonklusjonFor å finne den mest hensiktsmessige administrasjonsmåten av legemidler til barn er sykepleierne avhengig av god kommunikasjon, og godt samarbeid, med pasient, foreldre og lege. Sykepleieren må være kjent med ulike legemiddelformer, og hvilke administrasjonsmåter som egner seg utfra barnets alder og tilstand. For å gi best behandling er sykepleieren avhengig av kunnskap om syke barn og hvordan de skiller seg fra syke voksene. Tilstrekkelig bemanning, både grunnbemanning og innleid hjelp, er viktig for å forhindre at tidspress negativt påvirker sykepleierens arbeid med administrering av legemidler. Abstract
Introduction/purposeChildren in the Emergency Room (ER) often suffer from pain and anxiety. In some cases, this presents the need for medicinal treatment. Nurses administer medicines, which can vary in routes of administration. The purpose of this study is to investigate which factors nurses in the ER experience influence the decision of route of administration to children.
MethodThis project used a qualitative method with a phenomenological approach. It was carried out by two students. Individual semi-structured interviews took place, with a total of four nurses employed at a children’s ER. Prior to the interviews, informational letters were sent out, and the interviews were conducted with the help of an interview guide. The interviews were written down and analyzed with inspiration from Dalland's method of analysis and interpretation, and thematic processing.
ResultsNurses experienced communication and collaboration, pharmaceutical forms, patient situation, and time pressure to be factors that influenced which route of administration was chosen. The nurses communicated and collaborated with the patient, parents, and doctor to find the solution best suited the individual patient. Which pharmaceutical forms that were available, and the patient's condition, influenced which routes of administration were deemed appropriate. Time pressure caused by staffing issues, patient arrivals, and developments in digital systems, also affected the nurses' work in the selection process.
Conclusion To find the most appropriate route of administration to children, the nurses depend on good communication and cooperation with the patient, parents, and doctor. The nurse must be familiar with different pharmaceutical forms, and which routes of administration are suitable based on the child's age and condition. To provide the best treatment, nurses depend on knowledge of ill children and how they differ from ill adults. Sufficient staffing proves important to prevent time pressure negatively affecting the nurse's work with administration of medicines.
