Hvordan kan sykepleier styrke anoreksipasientens autonomi under tvangsbehandling?

Sammendrag

Hensikt

Lov om psykisk helsevern sier at gjennomføring av tvang innen psykisk helsevern skal skje på en forsvarlig måte og være i overensstemmelse med pasientens behov og medbestemmelsesrett (psykisk helsevernloven, 2001). Som sykepleier i tvangsbehandlingen av anorexia pasienter kan dette synes vanskelig, da pasienten har begrenset medvirkning rundt egen behandling. Vi opplever at sykepleiere har ulik kunnskap og praksis om hvordan dem involverer pasienten. Hensikten med oppgaven er å belyse de ulike aspektene ved autonomi under tvangsbehandling, og se hvordan sykepleier kan styrke, motivere og ivareta autonomi under behandling av anorexia pasienter.

Metode

Ni artikler om tema ble funnet ved hjelp av systematiske søk i databasene Pubmed og Cinahl. Artiklene ble screenet og resultatene syntetisert for å svare på utformet problemstilling.

Resultat

Tillitsfulle relasjoner og god autonomistøtte fra helsepersonell, venner, familie og medpasienter hadde i høy grad positiv effekt på pasientens egenmotivasjon og autonomi under tvangsbehandling. Sykepleier vil møte både muligheter og utfordringer i arbeidet med disse pasientene. Sykepleiers evne til å vise forståelse, ta seg tid, og ha kunnskap om anoreksi pasienten, er avgjørende for hvordan man kan legge til rette for å styrke pasientens motivasjon og autonomi.

Konklusjon

Ved å belyse de ulike aspektene ved autonomi, vil dette være med å styrke sykepleiers kompetanse og rolle rundt hvilke muligheter anoreksi pasienten har til brukermedvirkning under tvang. Dette innebærer at sykepleier motiverer, legger til rette og skaper et tillitsfullt forhold til pasienten. Dette styrket pasientens autonomi og troen på videre behandling. Objective

The mental health care act states that the implementation of coercion in mental health care should be carried out in a responsible manner and in accordance with the patient’s need and right to self-determination. As a nurse involved in the coercive treatment of Anorexia Nervosa, this can be challenging as the patient has limited involvement in their own treatment. We have observed that nurses have varying knowledge and practices on how to involve the patient.

Method

Nine articles on the topic were identified through systematic searches in the databases Pubmed and Cinahl. The articles were screened and the results synthesized to address the formulated reseach question.

Results

Trustworthy relationships and good autonomy support from healthcare professionals, friends, family and fellow patients had a significant positive impact on the patient’s selfmotivation and autonomy during coercive treatment. Nurses will encounter both opportunities and challenges in working with these patients. The nurse’s ability to show understanding, take time and have knowledge about the anorexia patient is crucial in fascilitating the strengthening of the patient’s motivation and autonomy.

Conclusion

By shedding light on the various aspects of autonomy, this will contribute to strengthening the nurse's competence and role in understanding the opportunities for user participation by anorexia patients during coercion. This entails that the nurse motivates, facilitates, and establishes a trusting relationship with the patient. This strengthens the patient's autonomy and belief in further treatment.
