Assessment of sleep length in older adults: comparison of self-reported and device-based methods to the radar-based Somnofy® sensor

Abstract
Mål: Målet med denne masteroppgaven er å vurdere nøyaktigheten av aktigrafi, akselerometre og søvndagbøker for å måle søvnlengde hos eldre, sammenlignet med den radarbaserte Somnofy-sensoren.Metode: 30 eldre voksne (50% kvinner) hadde Somnofy plassert på soverommet sitt, brukte Actiwatch og to Axivity AX3 akselerometre og fylte ut søvndagbok i 2 uker. Forskjellene og nøyaktigheten til de ulike målemetodene ble testet opp mot Somnofy data. Dette ved å sammenligne gjennomsnitts-/medianverdier, korrelasjon, kvadratisk gjennomsnitts feil og Bland-Altman-plots. Bland-Altman-plottene ble presentert ved å bruke gjennomsnittsfeil og «limits of agreement» (LoA).Resultater: Fra gjennomsnittsminuttene med søvnlengde/tid i sengen for alle deltakerne var det en forskjell på 22,78 minutter for Actiwatch-data, 104,09 minutter for Axivity AX3-data og 1,76 minutter for søvndagbok-data sammenlignet med Somnofy-data. Den sterkeste korrelasjonen (R=0,485) ble observert mellom Axivity AX3 (sov med partner) og tilsvarende Somnofy data, og den laveste RMSE-verdien (3,916) ble funnet for Actiwatch (alle) data. Bland-Altman-plottene viste at den minste forskjellen mellom metodene ble funnet mellom Actiwatch (sov med partner) data og tilsvarende Somnofy data (gjennomsnittsfeil 0,21 og 95% LoA 158,55 til -158,12), mens den største forskjellen ble observert mellom Axivity AX3 (sov alene) data og tilsvarende Somnofy data (gjennomsnittsfeil 145,41 og 95% LoA 375,12 til -84,30).Konklusjon: Resultatene var lovende for både Actiwatch og søvndagbok, mens nøyaktigheten til Axivity AX3 var tvilsom. Generelt viste søvndagboken de mest nøyaktige resultatene for måling av søvnlengde hos eldre på gruppenivå. Det er imidlertid viktig å merke seg at den kliniske relevansen av disse funnene fortsatt er usikker. Videre studier bør sette søkelys på å sammenligne Axivity AX3 og Somnofy med ulike metoder, samt validere Somnofy mot polysomnografi for eldre. Aim: This master thesis aim is to assess the accuracy of actigraphy, three-axis accelerometers, and sleep diaries, in detecting sleep length in older adults, compared to the radar-based Somnofy sensor.Method: 30 older adults (50% women) had Somnofy placed in their bedroom, wore an Actiwatch and two Axivity AX3 accelerometers, and filled out a sleep diary for 2 weeks. The differences and the accuracy between estimations from Somnofy compared to the other methods were tested. This was assessed by comparing mean/median values, correlation, root-mean-square error (RMSE), and Bland-Altman plots. The Bland-Altman plots were presented as mean bias and limits of agreement (LoA).Results: From the mean minutes of sleep/time in bed for all participants there was a difference of 22,78 minutes for Actiwatch data, 104,09 minutes for Axivity AX3 data, and 1,76 minutes for sleep diary data compared to Somnofy data. The strongest correlation (R=0,485) was observed between Axivity AX3 (sleeping with partner) and corresponding Somnofy data, and the lowest RMSE-value (3,916) was found for the Actiwatch (all) data. The Bland-Altman plots revealed that the smallest difference between the methods was observed between the Actiwatch (sleeping with partner) data and corresponding Somnofy data (mean bias 0.21 and 95% LoA 158.55 to -158.12), whereas the largest difference was observed between Axivity AX3 (sleeping alone) data corresponding Somnofy data (mean bias 145.41 and 95% LoA 375.12 to -84.30).Conclusion: The results were promising for Actiwatch and sleep diary, while the accuracy of Axivity AX3 was questionable. In general, sleep diary showed the most accurate results for measuring sleep length in older adults at group level. However, it's important to note that the clinical relevance of these findings remains uncertain. Further studies should compare Axivity AX3 and Somnofy with various methods, as well as validate Somnofy against polysomnography for older adults.
