Abstract

Problemstilling: Hvor mange røntgen thorax blir foreskrevet uten tilstrekkelig indikasjon?

Hensikt: Hensikten med oppgaven er å samle inn data på hvor mange røntgen thorax som blir foreskrevet uten tilstrekkelig indikasjon. Dataene skal sammenlignes mellom fire ulike sykehus, og det skal sees på om dette er et problem på de ulike radiologiske avdelingene.

Metode: For å besvare problemstillingen best mulig er det valgt å bruke en kvantitativ metode, hvor spørreskjema benyttes til datainnsamling. En spørreundersøkelse egner seg godt til aktuell problemstilling, siden man kan tallfeste hvor mange henvisninger til røntgen thorax som blir foreskrevet uten tilstrekkelig indikasjon i løpet av en periode på fire uker. Til undersøkelsen ble det utarbeidet et enkelt spørreskjema med 6 avkrysningsmuligheter for ulike ikke tilstrekkelige indikasjoner. I tillegg skulle det også fylles ut om undersøkelsen ble utført eller ikke. For å anonymisere de ulike sykehusene er disse navngitt som sykehus 1, 2, 3 og 4 gjennom oppgaven.

Resultat: Det ble samlet inn 45 utfylte skjemaer totalt. Fra sykehus 1 ble det samlet inn 36 utfylte skjemaer, mens sykehus 2 fylte ut 9 skjemaer. Fra sykehus 3 og 4 ble det samlet inn 0 utfylte skjemaer. Av de totalt 45 utfylte skjemaene var hele 24.4% av henvisningene til røntgen thorax henvist som «rtg. thorax ved innleggelse». Alle de 45 undersøkelsene ble utført, til tross for at henvisningen ikke inneholdt tilstrekkelig indikasjon.

Diskusjon/konklusjon: Av de 45 røntgenundersøkelsene som ble utført til tross for at henvisningen ikke inneholdt tilstrekkelig indikasjon, kan man anta at kun 1.6 og 1.8 pasienter hadde behandlingsmessig nytte av røntgenundersøkelsen. Antakelsen bygger på resultatene til tidligere forskning som er nevnt i oppgaven. Etter spørreundersøkelsen kan mistanken om at det blir utført mange thoraxundersøkelser uten tilstrekkelig indikasjon bekreftes.

Thesis: How many chest x-rays are prescribed without sufficient indication?

Purpose: The purpose of this thesis is to collect data on how many chest x-rays are prescribed without sufficient indication. The data will be compared between four different hospitals, to investigate if this is a problem in various radiological departments.

Method: To best answer the problem, a quantitative method is chosen where questionnaires are used for data collection. A survey is well suited to the current thesis, since it can quantify how many references to chest x-rays are prescribed without sufficient indication within a period of four weeks. A questionnaire was made with 6 check options for different indications. One of the questions was also if the exam was performed or not. To anonymize the various hospitals, these are mentioned as hospital 1, 2, 3 and 4 through the assignment.

Result: A total of 45 completed questionnaires were collected. From hospital 1, 36 completed forms were collected, while hospital 2 filled out 9 forms. From hospitals 3 and 4, 0 completed forms were collected. Of the total 45 completed forms, as many as 24.4% of the referrals to chest x-ray were referred as «chest x-ray upon admission». All 45 investigations were carried out, even though the referral did not contain sufficient indication.

Conclusion: Of the 45 x-ray examinations performed despite the referral not containing sufficient indication, we can assume that only 1.6 and 1.8 patients had treatment benefit from the x-ray examination. The assumption is based on the results of previous research mentioned in the thesis. After the survey, the suspicion that many thoracic examinations are performed without sufficient indication can be confirmed.
