Sammenligning av to ulike kassetter for oppsett av blodtypeantistoffscreening på analysemaskinen Ortho Vision Max
Abstract
Man kan danne blodtypeantistoff mot fremmede blodtypeantigener, dersom man gjennom graviditet eller blodtransfusjon blir utsatt for fremmede blodtypeantigener. Disse blodtypeantistoffene kan føre til hemolytisk transfusjonsreaksjon og hemolytisk sykdom. Blodtypeantistoffscreening utføres hos pasienter som skal ha blodtransfusjon og gravide, mens blodtypeantistoffindentifisering utføres dersom det har blitt utført en positiv blodtypeantistoffscreening. Analysen utføres ved bruk av kassetter, enten IgG-kassett eller poly-kassett. Hensikten med denne oppgaven var å undersøke om Anti-Humant globulin IgG-kassett kunne detektere og identifisere de klinisk viktige antistoffene, slik som Anti-Humant globulin poly-kassettene. I tillegg skulle det undersøkes om eventuelle uspesifikke bindinger som kan oppstå i poly-kassetten på grunn av komplement aktivering ble unngått ved bruk av IgG-kassett.
Det ble utført 29 blodtypeantistoffscreeninger og 5 blodtypeantistoffidentifiseringer. Disse ble utført med indirekte antiglobulinteknikk. Screeningene ble utført på instrumentet Ortho Vision Max, mens identifiseringene ble utført manuelt ved hjelp av Ortho Workstation. Det ble benyttet EDTA-plasma som inneholdt ulike irregulære blodtypeantistoff.
Resultatene fra undersøkelsen viste at de irregulære blodtypeantistoffene stort sett ble detektert og identifisert i lik grad på begge kassettene. Det var noen små forskjeller på enkelte av prøvene, men de klinisk viktige antistoffene ble detektert og identifisert på begge kassettene.
Siden begge kassettene detekterer og identifiserer de klinisk viktige antistoffene, kan begge kassettene brukes til blodtypeantistoffscreening og -identifisering. Det er vanskelig å si noe om at bruk av IgG-kassett vil føre til færre uspesifikke bindinger uten klinisk betydning. 29 blood type antibody screenings and 5 blood type antibody identifications were performed. These were performed with the indirect antiglobulin technique. The results of the investigation showed that the irregular blood type antibodies were mostly detected and identified to the same extent on both cassettes. There were some small differences between some of the samples, but the clinically important antibodies were detected and identified on both cassettes.
Since both cassettes detect and identify the clinically important antibodies, both cassettes can be used for blood type antibody screening and identification. It is difficult to say whether the use of an IgG cassette will lead to fewer non-specific bindings without clinical significance.
