Hvordan kan sykepleiere i samhandling med pårørende bidra til god omsorg for palliative pasienter?

Abstract

Bakgrunn: Antallet dødsfall i sykehjem er økende, og sykehjemmene får stadig en viktigere rolle innen palliasjon. Alle sykehjem skal beherske palliasjon og sørge for ivaretakelse ved livets slutt for sine pasienter og pårørende. Derfor er det nødvendig at helsepersonell ved sykehjem etablerer et godt samarbeid med pårørende.

Hensikt: Å utforske hvordan samhandling med pårørende kan bidra til god omsorg for pasienter ved livets slutt.

Metode: En systematisk litteraturstudie som benyttet fem forskningsartikler som ble funnet etter et grundig databasesøk og nøye utvelgelse av søkeord. Databasene som ble brukt til søket var Medline EBSCOhost, Cinahl Complete og Scopus. Artiklene ble funnet ved hjelp av søkeordene: Nurses/ Attitude of health personnel, Terminal care/ Palliative care, Nursing Homes, Family og Professional- family relations. Litteraturstudien bygger på forskning, litteratur og fagstoff.

Resultat: Etter analyse av relevante funn fra de utvalgte forskningsartiklene, var det tre kategorier som kom frem: «Ønsker og avgjørelser», «Forståelse og tilstedeværelse» og «Informasjon».

Konklusjon: En viktig oppgave for sykepleiere ved sykehjem er å etablere et godt samarbeid med pårørende. God samhandling er bygget på god kommunikasjon og relasjon, samt involvering, anerkjennelse og rolleavklaring med pårørende. Pårørende er viktige personer for pasienten, og kan gi omsorg og tilstedeværelse som helsepersonell ikke kan. De kjenner også pasienten best, og kan hjelpe med å tydeliggjøre pasientens ønsker. Informasjon, inkludering og ivaretakelse av pårørende til denne pasientgruppen har forbedringspotensial.

Background:The number of deaths in nursing homes are rising, and they get an increasingly more important role in palliative care. All nursing homes should be able to provide good care at the end of life for both the patient and family. It is necessary for healthcare personnel in nursing homes to establish good cooperation with the family.

Purpose: To explore how interaction with the family can contribute to good palliative care at the end of life.

Method: A systematic literature study who used five research articles. The articles were found after a thorough search through Medline EBSCOhost, CINAHL complete and Scopus. The following search-words were applied: Nurses/ Attitude of health personnel, Terminal care/ Palliative care, Nursing Homes, Family and Professional- family relations. This literature study is based off on relevant research and literature.

Results: After analyzing the findings from the research articles three main themes emerged: “Wishes and decisions”, “Understanding and presence” and “Information”

Conclusion: An important role for nurses in nursing homes is to establish good cooperation with the family. Good cooperation is built on good communication, relations, involvement, recognition and clarification of roles with the family. The family is important for the patient, and can give a type of care and presence that healthcare personnel can not. Besides, they know the patient best, and can help facilitate the patient's wishes. Information, inclusion and care for the family of this patient group has potential for improvement.
