Omsorg til døende under koronapandemien: en kvalitativ studie av sykepleieres opplevelse ved et hospice

Sammendrag

Innledning: Hospicefilosofien ligger til grunn for det palliative tilbudet i Norge. Omsorg er en viktig del av sykepleie til døende pasienter. Forskning viser at koronapandemien har belastet helsevesenet og satt barrierer for menneskelig kontakt. I denne studien undersøkes det hvordan sykepleiere på et hospice har opplevd å gi omsorg til døende under koronapandemien.

Metode: For å undersøke hvordan sykepleiere ved et hospice har opplevd å gi omsorg til døende under koronapandemien, ble det gjennomført fire kvalitative semistrukturerte dybdeintervju med sykepleiere ved samme hospice. Intervjuene ble gjennomført i mars 2022. Analysen har en fenomenologisk og hermeneutisk tilnærming, og er inspirert av Malteruds prinsipper for systematisk tekstkondensering.

Resultat: Resultatene fra denne studien viser at sykepleierne har opplevd hospicet som mer åpent og mindre belastende, foruten et smitteutbrudd som ble opplevd som tøff og belastende. Under koronapandemien har det vært utfordrende å gi omsorg i form av kroppskontakt. Sykepleierne har også opplevd i denne fasen at pasientene har blitt mer ensomme.

Konklusjon: I denne studien kan det tolkes at god bemanning og færre pasienter kan være en faktor til at sykepleierne på hospicet har opplevd koronapandemien mindre belastende. Sykepleierne opplevde at det å vise omsorg i form av kroppskontakt var utfordrende under koronapandemien. Smittevernregler og smittevernsutstyr opplevdes som barrierer for kroppskontakt. Under koronapandemien har sykepleiere opplevd at pasientene har blitt mer ensomme som et resultat av smitte i samfunnet og besøksrestriksjoner. For sykepleierne på hospice er det viktig å gi pasientene en verdig avslutning på livet. Derfor har de lagt til rette for å oppfylle pasientenes ønsker og behov.

Abstract

Introduction: The hospice philosophy is the basis for the palliative care offer in Norway. Care is an important part of nursing for dying patients. Research shows that the corona pandemic has burdened the health care system and set barriers to human contact. This study examines how hospice nurses have experienced caring for the dying during the corona pandemic.

Method: To investigate how nurses at hospice have experienced providing care to the dying during the corona pandemic, four qualitative semi-structured in-depth interviews were conducted with nurses at a hospice. The interviews were conducted in March 2022. The analysis has a phenomenological and hermeneutic approach and is inspired by Malterud's principles for systematic text condensation.

Results: The results from this study show that the nurses experienced the hospice as more open and less stressful, in addition to an outbreak that was perceived as tough and stressful. During the corona pandemic, it has been challenging to provide care in the form of body contact. The nurses have also experienced this phase and the patients have become lonelier.

Conclusion: In this study, it can be interpreted that good staffing and fewer patients may be a factor in the nurses at the hospice having experienced the corona pandemic less stressful. The nurses experienced that showing care in the form of body contact was challenging during the corona pandemic. Infection control rules and infection control equipment are perceived as barriers to body contact. During the corona pandemic, nurses have experienced that patients have become lonelier as a result of infection in the community and visitation restrictions. For the nurses, it is important to give the patients a dignified end to their lives, and therefore they are facilitated to have the patients' wishes and needs.
