Sammendrag
Introduksjon: Kognitiv og nevromuskulær svikt er noen av komplikasjonene som kanoppstå som følge av intensivbehandling. Tidlig mobilisering sikter på å forebygge ellerredusere disse komplikasjonene.
Hensikt: Å beskrive intensivsykepleiernes erfaringer med tidlig mobilisering avintuberte pasienter.
Metode: En kvalitativ studie med deskriptiv design og induktiv tilnærming, med semistrukturerte intervju av åtte intensivsykepleiere. Studiens tilnærming er inspirert avhermeneutikk og fenomenologi. Datamaterialet er analysert med hjelp av systematisktekstkondensering.
Resultat: Seks kategorier ble identifisert relatert til tidlig mobilisering av intubertepasienter; «hensynet til intensivpasienten», «forsvarlig mobilisering», «klinisk blikk»,«ressursmangel», «tverrfaglig samarbeid» og «standardisering av mobilisering».Intensivsykepleierne belyser at de mobiliserer med hensyn til pasientens rehabiliteringog livet etter intensivoppholdet. Intensivsykepleiernes yrkesrolle med sine ressurser ogkliniske vurderinger er av stor betydning for både tilrettelegging, organisering oggjennomføring av tidlig mobiliseringspraksis. Samtidig oppleves dyp sedering, mangelpå personell, tid og utstyr som en barriere for gjennomføring av tidlig mobilisering.Tverrfaglig samarbeid ble betraktet som en viktig ressurs. Dog er det en mangel påklare retningslinjer for hvordan mobilisering skal gjennomføres på en trygg ogforsvarlig måte.
Konklusjon: Tverrfaglig samarbeid og klare retningslinjer ses som nødvendig for tidligmobilisering av intuberte pasienter i en intensivavdeling. Hvilke barrierer som er tilhinder for tidlig mobilisering må kartlegges, og direkte adresseres for en suksessfullimplementering. Det er behov for ytterligere forskning for å få mer utdypet kunnskapom intensivsykepleiernes erfaringer med tidlig mobilisering av intuberte pasienter. Introduction: Cognitive and neuromuscular failure are some of the complications thatmay occur due to intensive treatment. Early mobilization aims to prevent or reducethese complications.
Aim: To describe the experiences of critical care nurses with early mobilization ofintubated patients.
Methods: Qualitative study with descriptive design and inductive approach inspired byhermeneutics and phenomenology. Semi-structured interviews of eight critical carenurses. The data is analyzed using systematic text condensation.
Results: Six categories were identified in relation to early mobilization of intubatedpatients; “in regards of the ICU patient”, “safe mobilization”, “clinical assessments”,“lack of resources”, “interdisciplinary cooperation” and “standardization ofmobilization”. The critical care nurses highlight that they are mobilizing with regards tothe patient's rehabilitation and their life after the ICU stay. The occupational role ofcritical care nurses with their resources and clinical assessments, is of greatimportance for both the facilitation, organization and implementation of earlymobilization practices. At the same time, deep sedation, the lack of personnel, timeand equipment is experienced as barriers to early mobilization. Interdisciplinarycooperation was considered an important resource. Nevertheless, there is a lack ofclear guidelines on how to mobilize in a safe and sound manner.
Conclusion: Interdisciplinary cooperation and clear guidelines are needed to earlymobilize intubated patients in the ICU. At the same time, it must be identified whichbarriers are preventing early mobilization, and directly address them for a successfulimplementation. Further research is needed to gain more in-depth knowledge of criticalcare nurses’ experience of early mobilization of intubated patients.
