The accidental statistics from 2003 to 2013 revealed that the frequency of floating production storage and offloadingshuttle tanker (FPSO-ST) collision risk was significantly above the cut-off limit. There is a need for further risk reduction, especially related to incidents with dynamic positioning (DP) system. The objectives of the paper are to identify the most basic factors underlying FPSO-ST collision and to provide the basis to eliminate DP accidents and incidents in the future. MTO (Man, Technology and Organization) analysis was applied to investigate the root causes and barrier failures of 9 reported accidents and incidents occurring over a 16-year period (2000-2015) in the North Sea. The results represent the event sequence of each accident and incident together with their immediate causes, root causes as well as failed or missing barriers including technical, human and organizational aspects. An interesting finding is that all of the 9 DP system accidents and incidents are due to a combination of risk influencing factors (RIFs). Suggestions in terms of effective prevention measures of FPSO-ST collision risk are provided in the conclusion section.
