Bruk av individuell plan i palliativ omsorg
Abstract
Innledning: Individuell plan blir anbefalt som verktøy for å ivarta tverrfagelig samarbeid, og sikre overganger på tvers av tjenestenivå. Individuell plan er til tross for dette lite brukt, og vi har jobbet ut i fra følgende problemstilling: «Hvilke faktorer påvirker bruk av individuell plan til pasienter i palliativ fase og hvilken betydning har en slik plan i samhandlingen mellom de forskjellige aktørene?».
Metode: Vi har gjort en litteraturstudie, der vi har søkt etter vitenskapelige artikler.Vi har kommet frem til et utvalg på 6 artikler, etter å ha vurdert deres relevans, pålitelighet og gyldighet.
Resultat: Vi har kommet frem til følgende faktorer som påvirker bruk av individuell plan: Organisering og bruk av individuell plan som verktøy, kompetanse og kunnskap, tverrfaglig samarbeid, ansvar, tid, og koordinators rolle. Det kan virke til at individuell plan er mest betydningsfull for å sikre overganger mellom ulike tjenestenivå. Ingen av artiklene er tydelige i hvorvidt individuell plan påvirker samhandlingen utover dette.
Drøfting: Vi ser at de forskjellige faktorene påvirker hverandre, og at spesielt kunnskap og kompetanse sees i sammenheng med alle.
Konklusjon: Individuell plan er lite brukt, til tross for at det er tydelig anbefalt som verktøy. Våre funn må sees i sammenheng med begrensninger i oppgaven som litteraturstudie, og valgte artiklers metode og informanter. Vi har sett faktorer som påvirker bruk av individuell plan. Hvorvidt individuell plan har betydning for samhandlingen, er fortsatt uklart. Introduction:. Individual plan is recommended as a tool to take care of interdisciplinary cooperation, and ensure transitions across service levels. Individual plan is despite of this, little in use, and we have worked out from the following issue: "What kind of factors is affecting the use of individual plan for patients in palliative care and how does this plan impact to the interaction between the different actors?".
Method: We have done a literature study, where we have searched for scientific articles. We have arrived at a variety of 6 articles, after assessing their relevance, reliability and validity.
Result: We have seen the following factors that affect the use of individual plan: Organisation and use of the individual plan as a tool, skills and knowledge, interdisciplinary cooperation, responsibility, time, and coordinators role. None of the articles are clear in whether it affects the interaction. It may seem that it is most significant to ensure the transitions between the various levels of service.
Discussion: We see that the different factors influence each other, and that particular knowledge has an impact for all of them.
Conclusion: The individual plan is in less of use, despite the fact that it is clearly recommended as a tool. Our findings must be viewed in the context of the limitations in the literature study, and the selected article method and informants. We have seen some factors that affect the use of the individual plan. Whether the individual plan is of importance for the interaction are still unclear.
